Street Reach Indy (SRI) Referral Instructions 

If you have a Street Reach Indy question, please reach out to: streetreach@chipindy.org 

  1. Complete this referral form and attach necessary documentation:
    • Completed budget
    • Sustainability plan
    • Homeless status documentation
    • W9 Vendor Identification Form for vendor being paid (if needed - see below for more information)
    • Explanation that supports financial assistance as it relates to the sustainability plan.
    • Current documentation (dated within 30 days) that outlines fees corresponding with funds requested.
    • Residential treatment documents as necessary (see below for more information)
  2. Street Reach Subcommittee will review and determine approval or denial.  The committee aims to make a decision within 72 hours, excluding weekends. 
  3. Requests may be submitted for the first month's rent or security deposit, but not both.
  4. If approved and all necessary documentation is collected, the Street Reach Indy referral will be submitted to CHIP's accounting department.
  5. Accounting department will follow up with the referrer for payment logistics as needed. Due to staff capacity, all checks are now required to be picked up. Please allow 5 business days for processing. 

Substance Abuse Treatment Request

  • Pending admittance: Include documentation confirming assessment date, date of entry and projected dates of residential treatment
  • Current admittance: Include invoice that outlines the dates of residential treatment.
  • Include W9 Vendor Identification Form completed by facility

W9 Vendor Identification Form

  • Tax law dictates that each vendor, receiving payment is provided a 1099 each year
  • Conditions that apply:
    • The amount spent with a vendor is over $600 for the year
    • The vendor is not incorporated (some LLC’s and similar organizations are exempt)
    • Individuals who contract to do work primarily receive 1099 forms as well

 Please note: Starting June 1, 2023, CHIP will be transitioning to HMIS for referral submissions.  You may begin submitting Street Reach referrals using the Street Reach Referral Form in HMIS any time.  Additional training and information will be forthcoming.
